Steps to recreate:
1) Open the add remote mysql instance page on dev-latest PMM server
2) Add username/password/host/port fields and select Use TLS for database connections checkbox
3) It will show the cert fields, fill some value in the cert fields
4) Change your mind and use the option to make only remote connection without using TLS, meaning uncheck the checkbox you selected on Step 2
5) Try adding the remote instance, you will see the request still tries to use TLS-SSL based connection, even when the user has not selected the checkbox.
Expected Behaviour : Ignore the value in the certificate fields, when user has unchecked the checkbox responsible for them
